offers a variety of different programs:

Click on a Program Name to Print an Application MA Transportation This program is for all clients that receive any form of Medical Assistance to help travel to and from doctor appointments. Also, to help assist with per diem and hotel expenses with prior authorization. Family Planning Waiver This program is for women ages 15-45 to help assist them with contraceptives and annual exams as needed. This program is based at 185% of the federal poverty level and is not available for women already receiving Medical Assistance. FoodShare(FS) This program is run through the state CARES system and helps families with nutritional burdens. FoodShare Employment Training (FSET) FSET is a program in the CARES system for individuals who receive food stamps and are mandatory to work for them. If no work is performed, this program allows the agency to sanction them from a monthly benefit. Kinship CareThis program is for children under the age of 18 that live with a relative caretaker. The program helps the relative caretaker with financial responsibilities by receiving a monthly benefit. Wisconsin Shares W-2 Child CareThis program is for childcare services that family is 185% of the Federal Poverty Level. Childcare Development Funds (CCDF) this program is for childcare services for enrolled Tribal children that's family is below 235% of the Federal Poverty Level. CCDF funds are also used to support community involvement and promote educational activities. Temporary Assistance to Needy Families (TANF) This program is for enrolled adult Tribal members residing in Forest County and for families on the reservation with enrolled Tribal children. General Assistance(G.A.) This program is for any Tribal member of a federally recognized tribe living on the Sokaogon Chippewa Reservation. Native Employment Works (N.E.W.) This program is for enrolled Sokaogon Chippewa Tribal members in Forest County. This program is intended to help clients maintain or gain employment. Workforce Investment Act (W.I.A.) Formerly Job Training Partnership Act (JTPA) this program is a job-training program that is for Native Americans with employment barriers. This program is also based upon income eligibility. Wisconsin Home Energy Assistance Program (WHEAP) This program is for people that are below 150% of the Federal Poverty Level. It assists with heating payments and furnace repairs. Weatherization This program is a referral program-if the client is eligible for WHEAP, they are eligible for Weatherization, every 5 years. Keep Wisconsin Warm Funds (KWWF) Non-profit energy assistance for elderly, disabled, or families with young children. |
